The Process
1. Care Consultation
Getting the best level of care starts with a personal assessment. All consultations are mainly done over the telephone or virtually. When you contact Heart2Care Support Services, the goal is to learn about your needs, established routines, and desires in order to see how I can best support you. During this time you will also have the opportunity to get answers to any questions you may have. Based on the consultation, a in-home visit may be arranged to conduct a basic needs assessment.
2. Established A Care Profile
After the care consultation is completed, a personalized care plan is developed that is tailored to your specific needs. The plan incorporates your activities of daily living routines and preferences in order to create the support that will be the most beneficial. I will review the care plan with you in detail and modify as needed to ensure you receive the kind of care that truly fits.
